Problem Statement on case Dylan Barton*
42 year old white male presents with frequent, small volume episodes of watery diarrhea x 3 days with abdominal pain and cramping rated 8 out of 10. Reports six or more diarrhea episodes daily with nocturnal diarrhea and blood-streaks in stool for 1.5 days. Sx are interfering with daily activities including work. Recently returned from work travel where he ate undercooked red meat with onset of abdominal pain and diarrhea upon return home. Has taken OTC bismuth without relief. If febrile with dry mucous membranes and diffuse abdominal tenderness, no guarding. DRE revealed stool positive guaiac. Skin turgor is intact but mildly diaphoretic and can tolerate oral intake. Has no known sick contacts and family with similar symptoms. No recent antibiotic use. Reports hx of bouts of diarrhea. No family history of similar illness. Reports ETOH intake 3-4 nights/week and denies tobacco use. Denies other pertinent health history.
