Problem Statement on case Jimmy Duran
Patient is here to establish care and for a 5 year-old wellness check. Patient is meeting developmental milestones, and exam shows a healthy little boy with no worrisome symptoms or exam findings. Patient missed 4 year-old wellness check and dental cleaning and will need DTaP-5th dose, IPV-4th dose, MMR-2nd dose, VAR-2nd dose, and annual dose of influenza vaccine. Exam shows good dental hygeine, but will need Fluoride treatment.
