Problem Statement on case Karen Rivers*
KR is a 57-year-old Caucasian female with a past medical history of CAD s/p stent to RCA and unsuccessful LCA balloon angioplasty (10 days ago), hypertension, hyperlipidemia, GERD, obesity (BMI 29.3), long-term smoker (a pack and a half for 25 years, recent cessation after heart attack)) who presents with the chief complaint of chest pain associated with nausea. Patient notes that this new episode of chest pain is different from when she had a heart attack 10 days ago, or heartburn. Patient states that her chest pain radiates to her back when she takes a deeper breath. Patient reports leaning forward helps relieve chest pain but gets much worse when lying down, moving, and with deep breathing. Physical examination revealed increased chest pain with inspiration, pericardial friction rub, fever of 101.3 F, and normal jugular venous pressure. Patient states that she takes medication as prescribed.
