16 year old female presents to the clinic aaox4. Patient was brought my her mother for concerns of 30 lb weight loss over past year and 20 lb weight loss over last six months. Mother states patient is eating less and less and exercising excessively. Mother reports patient fainted during volleyball practice because she hadn't eaten all day. Mother reports patient worries how others sees her, decreased social contact, and anxious at times. Pt reports she feels like her mother is always concerned about her eating and weight and she will stuff food in her shirt sleeves to keep her mother from worrying.
