The patient reports no medical history, does not take any prescription medications, but is taking over-the-counter ibuprofen 800 times a day with no relief. The patient endorses recently beginning a sudden, vigorous exercise program after being sedentary; he reports no recent changes to his diet. Physical examination reveals large muscle tenderness of the hamstrings and quadriceps, spinal extensor tenderness, hypertension, and orthostatic lightheadedness. The patient reports that his father is healthy, and his mother has obesity and diabetes mellitus.
