Problem Statement on case Robert Williams

Mr. W is a 55-year-old man who arrives in the emergency department complaining of a severe, episodic, rapid-onset epigastric pain radiating across the abdomen under his right ribs. Patient describes the pain with a severity of 8/10 associated with a recent rapid onset of nausea and vomiting. He had resolved 5-6 episodes in the past 3 months without prescription. His past medical history is remarkable for controlled hypertension and resolved appendectomy at age 7. On exam, patient is orthostatic hypotensive. Abdominal exam reveals moderately obese on inspection, pain to deep RUQ palpation, positive mild guarding, and hyperactive bowel sounds in 4 quadrants.

Remainder of abdomen is nontender. No noted rebound tenderness, herniation, or murphy's sign. Stool is guaiac negative.
