Problem Statement on case Vanessa Barr*
V.B. is an 18 y/o white female admitted to the clinic for acutely progressive dyspnea x 2 hours, increasing symptoms over the last one to two weeks. Severe progressive, nontraumatic, midthoracic back pain x 2 weeks without tenderness to palpation. The pain is constant. The patient had been on Prednisone, and it did help for about 3 days, but it stopped. She has been using Acetaminophen and Ibuprofen with no relief. The pain does not radiate. The pain is a 10 out of 10 and sharp in nature. She c/o night sweats, malaise, and orthopnea. Physical exam revealed PMI displaced toward sternum. Fixed 2cm supraclavicular LN with tracheal deviation. Diminished left chest rise. Denies history of drugs, alcohol, and smoking. No family history noted.
