Problem Statement on case Walter Sobchak
This is a 48-year-old male with a PMH of Hypertension, peptic ulcer disease, chronic tension headaches, and unclear episode of kidney injury also chronic 30 pack-year cigarette smoker, Alcohol drinks on average six beers each weekend, and non-complaint with his medication presenting with complaints of two-week shortness of breath associated with fatigue and anorexia. He reported having worsened dyspnea during exertion as well as 2-pillow orthopnea, He noted symptom of generalized skin itchiness without evidence of a rash. On physical exam Significant bilateral lung fields dullness on percussion Bilateral 2+ pitting edema to knee-level and BP 180/112mmHg.
He denies wheezing, cough, sputum production, hemoptysis, fever/chills.
