KM is a 13-year-old male patient who presents to the ED today with sudden onset of pain in the left scrotum 2 hours ago while playing basketball. He has had associated nausea with one episode of vomiting since the onset of pain. He has had no past similar occurrences. His physical exam is positive for pain and agitation. He is tachycardia and diaphoretic but afebrile. His genital exam is positive for left scrotal tenderness, swelling, and redness, with an absent cremasteric reflex ipsilaterally.
