Brianna Williams is a 36-year-old pregnant patient who is on her 38th week of gestation presented to the ED due to dyspnea on exertion and at rest for the past two days. Her symptom is associated with cough, wheezing and palpitations with reports of weight gain and increasing BLE edema. Her medical history includes gestational hypertension and is currently taking methyldopa as prescribed. Upon presentation, she was noted to be in respiratory distress, diaphoretic, and anxious. Her oxygen saturation was noted at 90% in room air, heart rate at 114 bpm and respiration at 22 per minute. Physical examination revealed inspiratory and expiratory wheezing, late systolic murmur, laterally displaced apical impulse (PMI) to midclavicular line and BLE pitting edema.
