Walter Sobchak is a 48-year-old male patient presenting with complaints of two-week shortness of breath associated with fatigue and anorexia. He reported having worsened dyspnea during exertion as well as 2-pillow orthopnea. The patient is a chronic 30 pack-year cigarette smoker with a medical history that includes hypertension, peptic ulcer disease, chronic tension headaches, and unclear episode of kidney injury. He admits to not adhering with his prescribed treatments. He also noted symptom of generalized skin itchiness without evidence of a rash.

 

Significant physical examination findings include BP of 180/112, auscultated crackles with dullness on both lung bases, and bilateral 2+ pitting edema to knee-level.
