My name is Babita. I am a nurse practitioner student, I will be taking your health history interview and performing a physical exam with my preceptor it is ok with you? MK: Hello, I am doing fine. That’s ok. NP: What brings you in today? MK: I am here for my physical done. NP: when was the last time you had physical? MK: It’s been a long time; I didn’t have health insurance. Now I got job in Walmart, have insurance, so wants to get it done. NP: congratulation and good luck for your new job. What kind of job you had before? MK: I used to work in Indian store, I lost the job due to pandemic. NP: Sorry to hear about that. Pandemic is making people’s life hard. Tell me about your overall health. What should you want to discuss with us today? MK: No, nothing really, current situation is making life hard you right, I am not able to go back to gym its still closed. I tried to walk when I am not working. but it’s hard for me to follow diet, my sleeping habit isn’t consistent. home during the pandemic is such a challenge for me. NP: I hear you. I am totally agreed with you. Your records say you are 143 lbs. today and 5’3’’ tall correct? MK: Yes, it is. I have gained 10 lbs. in 3 moths. -NP: Let me see your BMI, its 25.3 KG/m2 Which is at the borderline. Is there anything else contributing for wight gain beside you had mentioned above. -MK: No, I don’t think so. -NP: Tell me about your eating habit? -MK: I do eat out a lot, I live alone, so its time consuming to prepare meals and also my work scheduled is different every day. I usually . eat fast food and drink soda. Which isn’t good for me. -NP: I am glad you understand fast food and sodas are not healthy food. Main important steps are lifestyle change, by choosing healthy diet, lots of fruits and vegetables adding in meal. We can guide you in this journey. Would you say do you have stressful last year? -MK: Most definitely. -NP: Please tell me more about it. -MK: I lost my job, had financial difficulties, my aunt and grandfather die from COVID. My Family had rough time. It has been difficult year. -NP: I am very sorry for your loss; you had such a bad year. -MK: Thank you. -NP: Are you a stress eater? You eat more in stress? -MK: Thanks for bringing that point out. Yes, I do. When my ant and grandma died, my partner left for another girl. I was devastated. I did not come out of the house for two weeks. I was binging on chips, cake, and soda. I gained so much weight that time. NP: How was your relationship like before the breakup? -Mk: He was cheating with me, we fought often. Then we made up. My family rejects me; society rejects me.
