CC (chief complaint): “I am okay, I just need my meds”

HPI: E.T is a 37-year-old African American female with a history of schizophrenia, depression, and anxiety who presents to the clinic for initial psychiatric evaluation. The patient reports she was referred to the clinic by Houston behavioral hospital for suicidal ideation. She states she was recently hospitalized at Houston behavioral hospital for suicidal ideation. She states she called 911 herself when she had Suicidal ideation and she was taken to Houston behavioral hospital where she was admitted on 2/7/22 and was discharged on 3/11/22. She states she was diagnosed with schizophrenia and was prescribed Ability 10 mg PO daily, Seroquel 400 mg 2 tabs at bedtime, and Trileptal 600mg PO at bedtime. She states she was also diagnosed with schizophrenia when she was 25 years old. She reports her symptoms ( hallucinations, delusions, anxiety, and depression) are worse without her medications and she is currently out of her medications. She states her symptoms are well controlled when she is on her medications. She reports she has never been symptom-free for any particular period. She complains of sleep disturbance, anxiety, and depression, but denies changes in appetite. The patient denies any current SI/HI/AVH……….. Continue
