HPI: Patti, a 40-year-old Iranian female client presents with her 23-year-old daughter (Sharleen) along with their therapist (Sandi) for a family psychotherapy session with a consultant therapist (Gonzalo). The family’s therapist (Sandi) requested the assistance of the consultant therapist because of worsening family relationships. Sandi along with Patti and Sharleen explain that there has been chaos in the house about two weeks after Patti’s third child (21-year-old daughter Shireen) who had just joined them in the United States from Iran two years ago. Shireen had revealed to the family that she had been sexually abused by their father, Patti’s husband, as well as the blame on Patti for leaving her behind to suffer the ill treatment she got from her father. This caused chaos to erupt in the home with constant yelling and cursing, as well as screaming. Patti migrated to the United States with four of her five children about 12 years prior, leaving behind one child (Shireen) who was eight years old at the time because the US Embassy could not grant her a visa. Patti also feels detached from her children, and that she is no longer in charge, and has feelings of no longer being needed by her children. Patti expresses feelings of loneliness and depression, feeling hopeless and helpless. Especially since her failed foot surgeries that has left her disabled, unable to walk or work. Patti also expresses that she is in constant pain, especially in her back.
