Chief Complaint: “I’m doing pretty well.”

History of Present Illness: Patient is a 13-year-old White adolescent male who presented voluntarily with major depressive disorder and a history of attention deficient hyperactivity disorder (ADHD) and autism. He was accompanied by his mother for a follow up on medications and for psychotherapy group classes and activities.

Past Psychiatric History Hospitalizations: Patient was sexually and physically abused by his biological father. Patient was hospitalized twice in the past due to suicidal attempts, one admission last year and at the beginning of this year. Patient has history of self-inflicted injury by cutting his arms with sharp knife and banging his head against the wall.

Family Psychiatric/Substance use History: Denies family history of psychiatric illness. He is the youngest in the family genogram, father at the top of the triangle, stepfather on the opposite side, mother at the base, and his sister and him on both sides of the genogram. No one has endorsed any psychiatric history. MB was born and raised by his parents.

Medical History: Denies medical history.

Psychosocial History: Patient was born and raised by his biological parents until he was 8 years old. He now lives with his mother, stepfather, and older sister. Patient is a student in the 8^th grade. Denies medical problems, denies alcohol, or drug abuse. Caregivers are his mother and older sister. Patient is in middle school and is home schooling. His hobbies mostly include video games. He is not working nor has any paid job. Denies any legal history both past and present……….. Continue
