The client in this case study is a 42-year-old man presenting to the ED with a 2-day history of low back pain, dysuria, severe perineal pain accompanied by fevers and chills, and inability to empty his bladder fully. Per the patient, the pain is worse when he stands up and it is somewhat relieved when he lies down. Vitals are a temperature of 104.0 F, a pulse of 138, respirations are 24, and oxygen saturation is 96% on room air. The digital rectal exam (DRE) reveals that the prostate is enlarged, extremely tender, warm to the touch, and swollen. This paper will analyze this case study and explain the factors that affect fertility, why inflammatory markers rise in STD/PID, why prostatitis and infection occur, and the causes of a systemic reaction.
Factors That Affect Fertility
When infected with an STI/PID, there are factors that affect fertility.
According to the CDC, “Chlamydia and gonorrhea are important
preventable causes of pelvic inflammatory disease (PID) and infertility”
(2023, para. 1). For men, untreated urethral infection can potentially lead
to sterility (McCance and Heuther, 2019). For women, sequelae of STIs
include adverse birth outcomes such as stillbirths, and some bacterial
STIs cause PID, preterm delivery, low birth weight, and female infertility
