Pt is a 16 year-old hispanic female with cough x 3 weeks that is worsening, and worsening SOB beginning 3 days ago. Patient states coughing is worsening at night and keeps her up. Patient states SOB scares her, seems to be exacerbated with exercise and relieved with rest. Has SOB accompanying cough at night. Pt's O2 is 95%, and resp rate of 22 breaths/minute, regular and unlabored with subcostal retractions. Upon examination wheezing found in all lung lobes bilaterally. Pt had URI 3 weeks ago with resolved nasal congestion and sore throat after 1 week. Negative for fever, and other URI symptoms. hx of eczema and reactive airway disease. Reports roaches and exposure to secondhand smoke. reports anxiety. No insurance.
