Comprehensive Physical Assessment – Head to Toe Patient Initials: TJ Age: 28 Gender: Female

SUBJECTIVE DATA:

 

Chief Complaint (CC): “I came in because I'm required to have a recent physical exam for the health insurance at my new job”.

HPI: The patient has secured a new job. As part of the workplace routine, she is required to get a pre-employment physical assessment before she can be fully employed. Therefore, she did not come to the clinic because of a specific concern. Nonetheless, she admits to having visited a hospital about four months ago to have her yearly gynecologic exam. It is then that she got diagnosed with the condition referred to as the polycystic ovary syndrome. The healthcare professional prescribed some medicine for her. Apart from this, the patient has also been diagnosed with type 2 diabetes. She conducts regular exercises, watches her diet, and uses Metformin to control the disease. She first started using Metformin about five months ago and has not reported any side effects. Altogether, she feels she is healthy and ready for her new job.

Current Medications: Acetaminophen 50 mg PO prn (Headache); Albuterol 90 mcg/spray MDI 2 puffs Q4H prn; Fluticasone Propionate 110 mg 2 puffs BID (Nasal symptoms); Ibuprofen 600 mg PO TID prn (Menstrual cramps); Metformin 850 mg (Diabetes).

Allergies: Yes, to dust and cats; yes, to penicillin.

 

PMHx: She has had asthma since she was two and half years old. She has been using albuterol inhaler since. She has been hospitalized for the same once when she was in high school. As for

 

type 2 diabetes, it was first diagnosed when she hit 24 years. She has to stay healthy by keeping fit and monitoring her blood sugar level at 90. As well, she uses Metformin. Another condition she has ever been diagnosed with his high blood pressure. No surgeries so far........... Continue
