Patient is a 16 year old female complaining of burning upon urination with increased frequency, strong odor and abdominal cramps although not on menstrual cycle. States her last menstrual cycle was 3.5 weeks ago and she should be starting her cycle again soon. Admits sexual activity with one partner (boyfriend). Boyfriend uses condom and she is currently taking birth control. Admits increased sexual activity the past week. Admits symptoms may be due to forgetting to urinate after last sexual activity. States pain started/increased 3 days ago. Denies fever, fatigue, malaise and back pain. Denies vaginal discharge. States she drinks close to 64 oz of water per day as recommended by her cheerleading coach.
