HPI: H.S is a 25- year- old white female with a history of ADHD and
Anxiety disorder who presents to the clinic today for initial psychiatric
evaluation. The patient reports she has been diagnosed with anxiety and
ADHD and was in therapy back in Utah. She states she was also
diagnosed with PTSD due to childhood trauma but declines the need for
any therapy for now regarding her PTSD. She states she just arrived in
Houston from Utah and currently living with her parents. She reports
symptoms of anxiety, worrying, depression, forgetfulness (forgets
important meetings), social anxiety, bad communication, irritability,
restlessness, trouble focusing, and concentrating at work (patient
misreads and misinterprets written instructions and makes mistakes). She
states her symptoms started in November of 2021 during her divorce.
The patient states she recently went through a divorce with her now ex-
husband in November of 2021 due to an abusive relationship. She states
she and her ex-husband were married for 6 years before the divorce.
The patient states she was hospitalized in November of 2021 because
she attempted suicide by swallowing multiple pills but didn’t specify the
names of the pills. She states she has never been symptoms-free for any
single period of time. She states she wants to replace her current anxiety
medication to manage her symptoms better……….. Continue
