CC (chief complaint): Her CC is " I am not doing that well "

History of Presenting Illness (HPI): A.B. is a 24-year-old African American female with a history of schizoaffective disorder, bipolar type, and schizophrenia who presents for initial psychiatric evaluation. The patient reports she was referred to this clinic by Houston Behavioral Hospital where she was recently hospitalized for Suicidal Ideation (SI), Auditory hallucination (AH), and paranoia. She was admitted to the hospital on 02/24/22 and discharged on 03/1/22.

She mentions that she hears voices, and the voices sound like people she

knows. She states she is still hearing voices especially when she is

around certain people. She also states the voices sometimes tell her she is

half-angel and half-demon; the accusatory voices are also telling her that

she will be arrested again; that she is going to hell and that her mom and

God are mad at her. She mentions she suffers from the effects of

childhood trauma; she states she was sexually abused by an older

female. She states she is not proud of some things she did in her past and

sometimes worries that the mistakes she made will catch up with her. She

states her symptoms started three years ago and are still ongoing. She

states she used to be in the assisted outpatient treatment (AOT) program

at Neuropsychiatric Center (NPC) 'a while back.' where they provided

her with food and helped her with other supplies, but she stopped the

program due to unknown reasons……….. Continue
