Introduction

Bipolar disorder is characterized by life-threatening shifts in mood levels. It is also known as manic-depressive illness because it involves episodes of mania and depression. During a manic episode, a person with bipolar disorder may feel euphoric, have an exaggerated sense of self-confidence, engage in risky behavior, and have difficulty sleeping (Tavormina, 2019). In contrast, during a depressive episode, they may feel sad and hopeless, have a lack of energy, and experience changes in appetite and sleep patterns. Bipolar disorder can be a hard condition to manage, but with proper treatment, many patients can live healthy and fulfilling lives. This SOAP note focuses on examining a 35-year-old Caucasian female who presents to the clinic with anxiety.

SUBJECTIVE

CC (chief complaint): “I’ve been all over the place, just anxious.”

HPI: The patient is a 35-year-old Caucasian female who complains of feeling anxious and all over the place. She reports having a lot of energy and cleaning a lot for the past four days, consistent with hypomania. The patient exhibits pressured speech and appears older than her stated age.
