Client is a 30 yr old female with a PPHx of Bipolar d/o, MRE Depressed and a PMHx of tonsillectomy in 2010 who presents for an initial psychiatric evaluation.
states, "i have struggled with mental health and substance abuse (mh for 10yrs and sa 7yrs). i started struggling with anxiety after i quit gymnastics. i started therapy and had an individual therapist who later diagnosed me with and was started on Lexapro by a psychiatrist. i later observed that i was having periods where i would be happy, feel good and energized for a couple of days and would later be depressed and would be in the depressive state for some time. i was diagnosed with bipolar 2 yrs ago and was started on Abilify. my Lexapro was recently d/c when i went to RCA and was started on Lamictal." states her meds are effective and well-tolerated. reports her anx and depression makes her drink EtOH and states her last use of EtOH was Feb 12, 2022. reports drinking 1L Vodka daily x 45 days prior to going to RCA for detox. denies withdrawal symptoms and states she has "little cravings" for EtOH.
