HPI: JB is a 15yo African American F here for psychiatric evaluation of DMDD with a history of ADHD, PTSD, and Fetal alcohol syndrome. The patient’s foster parent reports that the patient has been having increased aggression at home. The Foster mom reports that there was an incident where the patient threatened her with a knife after the patient was asked to wash the dishes, which resulted in a verbal altercation when the patient refused. The patient denied threatening her foster mom with a knife. She reports that she just “hates being told what to do.” The patient reports that she did not mean to threaten her foster mom and states that “the devil made me say it.” The patient also states that she has “anger issues.” “I refused to do the dishes and slammed the door and threw a basket at her.” She denies any intent to harm her foster mom. The patient currently denies having any SI/HI thoughts. The patient reports trauma from being sexually abused when she was ten years old and being neglected by her parents. The patient states she sometimes has nightmares regarding this trauma.......... Continue
