She reports that the recent changes to her mediction is working better for her” I am feling better ad I can get more done during the day, and I am as drowsy anymore. She even reports that she has been able to go outside, and able to complete activities of daily living. She reports her mood is better. I feel like I am starting to get out of the fog”.
HPI: Ms. R.F is a 36 yrs old single Caucasian female who is being managed for ongoing mental health disorders. She is diadnosed with bipolar disorder, agoraphobia, PTSD, and ADHD. Ms. R.F reports she’s feeling a bit better she says the hydroxyzine is helping her she is having a good restful night seep, and her symptoms of anxiety has decreased as well. She continues to take Prazosin 2mg and she’s having fewer nightmares. The Adderall was changed to Adderall XR 10 mg daily and she’s finding that she’s awake alert and able to accomplish most of her tasks during the day and she is sleeping well at night so she’s believing that the Adderall XR is working better for her. She has been going outside. She said she was outside for an hour last time, so the agoraphobia is getting better, and she continues to take propranolol. Ms. R.F has a history of high cholesterol and she’s on Zocor. She did mention muscle aches and pain. Ms. R.F was advised to call her PCP and see if they can make changes to her medication..........
