S: M.J.is is a 39-year-old customer who was referred to the facility by his doctor. He was referred for additional mental evaluation since the physician believed his health condition was not medical. The client stated that he felt hopeless in life and want to commit himself. His despondency sprang from his belief that he had failed to provide the best for his family. The individual was questioned more, and it was discovered that the emotions of despair lasted on most days throughout the day. He was also depressed on the majority of days. He also stated that he had difficulty falling asleep. His appetite has drastically decreased, resulting in a lack of energy on most days. He also admitted to having suicidal thoughts without making any arrangements. He stated that his ability to make judgments and concentrate had deteriorated dramatically in the previous month. There was no medical issue, medicine, or substance addiction to blame for the symptoms. As a result, he was diagnosed with serious depression and began therapy.

O: The patient seemed unkempt for the occasion. His speech was slowed down in terms of tempo and loudness. His self-reported mood was downbeat. Illusions, delusions, and hallucinations were disputed by the client. Throughout the evaluation, he kept constant eye contact. His intellectual content was forward-thinking.
